Shawn Long, MD, Chief Medical Officer
Dr. Long joined Mountain Laurel Medical Center in January 2015 as the Chief Medical Officer. Dr. Long is a board certified family physician and a member of the AAFP. He received a Bachelor of Arts in Biology in 1996 and his M.D. in 2000, both from West Virginia University in Morgantown, WV. Both his internship and residency were completed at the West Virginia University Department of Family Medicine. He was a faculty member at the WVU Department of Family Medicine in 2003 before taking a position with United Physicians Care at the state-designated Rural Health Clinic in Grafton, WV. There, he oversaw the development and growth of an after-hours clinic, as well as provided medical care to the community as a partner of the general family practice at Pinewood Medical Center. In 2011 he rejoined West Virginia University Hospitals as a provider at Cheat Lake Family Medicine, as well as the WVUH Wound Management and Hyperbaric Oxygen Center, where he served as a medical director until he joined Mountain Laurel. Dr. Long grew up in Weirton, WV.
Sarah Call, MD
Dr. Call joined Mountain Laurel Medical Center in June 2014. Dr. Call is a board certified family physician and a member of the AAFP. After high school she attended Boston College, receiving a Bachelor of Arts in History while she followed the pre-med track. She received her M.D. from Loyola University Chicago Stritch School of Medicine in 2007, graduating with honors. While in medical school she had several volunteering opportunities, including helping run a free medical clinic in the west side of Chicago and setting up a temporary medical clinic in the Dominican Republic for Haitian refugees. She completed her residency at Riverside Regional Medical Center in Newport News, VA. In her final year of residency, she served as Chief Resident and was part of the hospital’s ethics committee. She then joined a rural family medicine practice in central Massachusetts. Dr. Call was raised in St. Louis, Missouri. She and her husband, a veterinarian at Casselman Veterinary Services, are excited to have relocated to Garrett County with their two sons and daughter.
Robert Phares, MD
Dr. Phares joined MLMC in June of 2018. Dr. Phares is a family physician treating patients of all ages. He has a special interest in patients with complex medical conditions, diabetes, chronic pain syndromes, and weight management. He is a member of AAFP. Dr. Phares grew up in Morgantown, WV. After graduating from University High School, he attended West Virginia University. He received a Bachelor of Science degree in Secondary Teaching. He taught middle school for three years before going back to West Virginia University to follow West Virginia Medical School requirements. He continued to work with area youth programs throughout his undergraduate and medical school education. After graduation from WVU Medical School, Dr. Phares accepted a position at the Washington Hospital Residency Program in Washington, Pennsylvania. After serving as Chief Resident, he moved to Martinsburg, WV to start a family practice office in Hedgesville, WV. After 9 years of meeting this rural community medical needs, he moved back to his home place of Morgantown, WV. The last 5 years he owned and operated Bruceton Medical Center in Bruceton Mills, WV until his decision to join the staff at Mountain Laurel Medical Center. Throughout his career, Dr. Phares has served to accomplish a variety of community projects and has enjoyed using his teaching experience to educate his patients. Dr. Phares, his wife, Maggie, and three sons are excited to join the team that provides quality medical care here at Mountain Laurel Medical Center.
Daniel Buckingham, MD
Dr. Daniel Buckingham joined Mountain Laurel Medical Center in October 2016. He is board certified by the ABFM and is a member of the AAFP. After attending high school in Annapolis, MD, he graduated summa cum laude with a Bachelor of Science in Molecular Biology and Religion from Grove City College in Grove City, PA. An interest in international health and medicine took him to Israel for medical school. While there he received a strong foundation in population health and cross-cultural medicine. In 2002, he earned is M.D. from Ben Gurion University in Beersheva, Israel. Upon returning to the US, he completed his residency in family medicine at St. Elizabeth Medical Center in northern Kentucky. After serving as the Chief Resident, he did an additional year of fellowship training in surgical obstetrics and women’s health care at St. Elizabeth. After serving as a faculty member for the family practice residency at St. Elizabeth, he came to Garrett County. In private practice for ten years, he has delivered family medicine and obstetric care at Garrett Regional Medical Center, where he also served in numerous leadership roles. Being raised in a military family, he grew up in many different states. He and his wife Mandy, along with their ten children, are happy to call this area home.
Michelle Dixon, Director of Nursing, CRNP
Michelle joined Mountain Laurel Medical Center in June 2014 as the nursing supervisor. During her first year here, she decided to further her education and become a Family Nurse Practitioner. She obtained her Bachelor of Science in Nursing from Fairmont State University in 2012, and completed her Masters of Science in Nursing from Walden University in February 2018. She is board certified nationally by the American Academy of Nurse Practitioners and licensed by the state of Maryland. Prior to employment at Mountain Laurel she worked at Garrett Regional Medical Center. She is excited to be providing care to patients at this facility as a health care provider, as well as by continuing her current role as the Director of Nursing. She currently resides in Terra Alta, WV with her husband and two daughters.
Janelle Porter, PA-C
Janelle joined Mountain Laurel Medical Center in November 2014. Janelle is board certified nationally and by the state of Maryland as a Physician Assistant and is a member of the American Academy of Physician Assistants. She attended Indiana University of Pennsylvania Honors College where she studied Nutrition and minored in Biochemistry and Psychology. She worked as a Nursing Assistant in Pittsburgh before attending PA school. She received her Master’s degree in Physician Assistant studies from the University of Pittsburgh in 2012. After her training, Janelle relocated to Phoenix, Arizona where she started her career working at an inner city family practice office that served many refugee patients from all over the world. She traveled to Ensenada, Mexico during that time on a Medical Mission trip with a team sponsored by the Mayo Clinic. Missing the green hills and her family, Janelle decided to come back to the Eastern part of the country to continue practicing family medicine with Mountain Laurel Medical Center. She is passionate about primary care and excited to serve the community. Janelle was born and raised in Pittsburgh, PA. She is an avid Steelers, Penguins, and Pirates fan.
Lori A. Gregory, CRNP
Lori joined Mountain Laurel Medical Center in August 2016. Lori is board certified nationally by the American Academy of Nurse Practitioners as a Family Nurse Practitioner and licensed by the state of Maryland. After graduation from Southern High School, Lori received her Associate of Science in nursing in 1998 from Allegany College of Maryland, Bachelor of Science in Nursing from Frostburg State University in 2012, and Masters of Science in Nursing, Family Nurse Practitioner track, from Wheeling Jesuit University in 2016. She is a member of the American Association of Nurse Practitioners (AANP), Nurse Practitioner Association of Maryland (NPAM), and the Western Maryland AHEC Nurse Practitioner’s Caucus. Lori spent over 18 years working as a staff nurse and then as a patient care coordinator of long and short term care and rehabilitation services at Dennett Road Manor. Areas of healthcare experience include managing treatment needs of dementia, wounds and ulcerations, cardiovascular disease, and diabetes mellitus. She provides acute care, management of chronic diseases and preventative care. Lori is a Garrett County native. She and her husband, Jay, built their home in Oakland on her father’s farm where they raised three sons.
Jessica Meyers, CRNP
Jessica Meyers joined MLMC in May 2018 as a Family Nurse Practitioner. She is certified by the American Academy of Nurse Practitioners and licensed in the State of Maryland. Jessica is a member of the American Association of Nurse Practitioners (AANP) and is ACLS and PALS certified. Jessica was raised in Cumberland, MD and moved to Garrett County when she was ten. She is a Southern High School graduate and went on to complete a Bachelor of Science degree in Dietetics at Marshall University in 2007 (Cum Laude). After starting her career in nutrition counseling and meal planning, Jessica furthered her clinical education and returned to Allegany College of Maryland to pursue nursing and graduated from an accelerated nursing program in 2008. Her nursing experience started at Western Maryland Health system, with a focus in cardiovascular patient care. She then transferred closer to home and began her career with Garrett Regional Medical Center to work in positions of surgical services and post-anesthesia care; intensive care; Supervisor of cardiac & pulmonary rehab and clinical coordinator. These services provided her with experience in preventative health, acute care and chronic disease. After achieving her Master’s Degree from Walden University in 2017 (Cum Laude), she worked as a Nurse Practitioner in Urgent Care and the local Emergency Department. She currently resides in Oakland, Maryland with her husband, Bryson and her two children. Jessica looks forward to serving her community’s health care needs.
Barbara Monn, MS, PA-C
Barbara joined Mountain Laurel Medical Center in 2008. Barbara is board certified nationally and by the State of Maryland as a Physician Assistant. She is a member of the American Academy of Physicians Assistants. She attended Alderson Broaddus College, receiving a Bachelor of Medical Science in 1980 and her Masters of Science in 2007. Barbara began her career in Neurosurgery in Charleston, WV. She was then recruited by a local surgeon, Dr. Joseph Alvarez. After nine years in general surgery, she switched to Pediatrics with the Children’s Medical Group in Cumberland, MD. Striving for more experience and knowledge, she went to Mountain View Regional Medical Center, Health South in Morgantown, WV, where her focus was Physical Medicine and Rehabilitation. She then joined Dr. Margaret Kaiser and later, Barbara joined the Mountain Laurel family. She feels the varied fields that she has practiced, along with a strong educational background, provides her with a good basis to handle the needs of her patients. Barbara was raised in Garrett County. She has one son and lives near Oakland with her husband Kevan.
Taylor Crabtree, CRNP
Taylor Crabtree joined MLMC in April 2019 as a Family Nurse Practitioner. He is certified by the American Academy of Nurse Practitioners and licensed in the state of Maryland. Taylor is a member of the American Association of Nurse Practitioners and is ACLS certified by the American Heart Association. Taylor is a native of Garrett County. He graduated from Southern Garrett High School and completed an Associate’s Degree in Nursing at Allegany College of Maryland in 2010. His nursing experience started at Mon General Hospital in Morgantown, WV where he spent eight years working in the Intensive Care Unit. During that time he finished his Bachelor of Science in Nursing at West Virginia University. He then transferred to WVU Medicine-Ruby Memorial Hospital and worked as a Resource Pool Nurse in a variety of settings including cardiovascular, oncology, neurology, trauma, and general medicine units. Taylor graduated from West Virginia Wesleyan College in 2018 with a Master’s Degree in Nursing. Taylor resides in Oakland with his wife, Hannah, and their son, Henry. In his spare time he enjoys spending time outdoors, waxing his truck, and spending time with family. Taylor is excited to return home to provide high-quality patient-centered care to the citizens of Garrett County and the surrounding area.
Kimberly Maynard, CRNP
Kim joined Mountain Laurel Medical Center in June 2019 as a family nurse practitioner. Kim graduated from Allegany College of Maryland in 1999 and worked as a registered nurse in Cumberland before returning to school. She earned her Bachelor of Science in Nursing Degree from University of Maryland Baltimore and her Masters of Science in Nursing as a Family Nurse Practitioner from West Virginia University. She is board certified by the American Nurse’s Credentialing Center as a Family Nurse Practitioner. Kim is excited to be a part of Mountain Laurel Medical Center and looks forward to her serving the communities acute and chronic health care needs.
Jeannette Sweitzer, CRNP, Family and Pediatrics
Jeannette Sweitzer, CRNP, graduate of Marymount University and Davis and Elkins College, Jeannette is a dual-board certified nurse practitioner in both Family Practice and Pediatrics. She has been a health care provider in the Western Maryland area for over 25 years. Jeannette has joined Mountain Laurel to continue to provide quality health care. She especially enjoys finding new ways to improve the community by visiting local schools, senior centers, and various youth groups. She is also very active in the Boys and Girls Club of Eastern Panhandle. She currently resides in Berkeley Springs with her two teenage children but is a former resident of Garrett County and will be moving back to the area soon.
Sara Cockerham, LCSW-C
Manager of Behavioral Health
Sara Cockerham is a Licensed Clinical Social Worker with over 10 years of experience impacting quality of life for individuals and families in acute care, oncology care and the Health/Mental Health fields. Sara has significant training and experience working with those that are impacted by cancer and trauma throughout the life span. She provides psychotherapy to children, teens, and adults to improve an individual’s well-being and mental health, to resolve or mitigate troublesome behaviors, beliefs, compulsions, thoughts, or emotions, and to improve relationships and social skills. Sara has over 15 years of experience managing programs and teams to improve a lack of resources and address barriers to care and services in remote areas. Her unique experience working in diverse and remote areas has enhanced her ability to work with all individuals with a variety of mental health concerns in a primary care setting. Sara received her master’s degree in social work from the University of Alaska in Anchorage. She is a member of the Collaborative Family Healthcare Association (CFHA) and the National Association of Social Workers (NASW).
Sam Leizear, MSW-LGSW/LMSW

Sam Leizear, MSW-LGSW/LMSW joined MLMC as a Clinical Therapist/Behavioral Health Provider in February 2018, following a 36 year career in the Social Work Field which included work in Community Behavioral Health, a Center of Independent Living, all aspects of rural hospital social work, the WVU Center on Aging and the WVU School of Social Work. Mr. Leizear was raised in Damascus, MD, but relocated to Morgantown WV in 1978 to complete his undergraduate degree at WVU. After 15 years in social work practice he completed his MSW degree at WVU, and has been a WV licensed Social Worker since 1986. Although he enjoys working with all types of individuals, he has completed specialized practice experience, training , education and research in the areas of Aging, Domestic Violence, Trauma, Sexuality and Gender Identity, and Rural Health Access. He has also taught many hours of Diversity classes for WVU, as well as provided hundreds of hours of Continuing Education Trainings and Guest Lectures across a variety of settings. Mr. Leizear his wife (who is also a social worker), and their 4 dogs and 2 cats, reside in Morgantown, WV, and enjoy kayaking, adopting animals with special physical and/or medical needs; volunteer work with vulnerable humans and animals, trying new vegetarian recipes and reading.
Jessica Nice, MSW, LCSW-C
Jessica joined Mountain Laurel Medical Center in December 2014 as a Behavioral Health Consultant. She is a licensed Clinical Social worker in Maryland. Jessica attended West Virginia University where she received a Bachelor of Arts in Social Work in 2005. During graduate school Jessica worked for Valley Healthcare, providing supportive housing services to consumers with mental illness in order to enhance their ability to live independently. In 2007, she received her Masters of Social Work degree from West Virginia University, specializing in direct practice mental health. Upon graduation, Jessica was hired by the Maryland Department of Correctional Services, where she provided clinical, therapeutic, and forensic social work services in a criminal justice environment. Jessica provided individual and group counseling for inmates who needed rehabilitative intervention due to a max of behavioral, personality, and mental health factors. Jessica was born and raised in Terra Alta, West Virginia. She continues to live in the area with her husband and son.
Christy Lambert, Behavioral Health Provider
Christy Lambert joined Mountain Laurel in December 2018 as a Behavioral Health Provider. Christy is a licensed master social worker in the state of Maryland. She became certified as a Certified Clinical Trauma Professional (CCTP) through the International Association of Trauma Professionals in January 2019. She received her Bachelors of Science in Psychology and Bachelors of Arts in Communication in 2014 from the University of Pittsburgh at Johnstown and her Masters of Social Work and Graduate Certificate in Integrated Mental and Behavioral Health in 2017, from West Virginia University. Christy worked throughout her graduate studies at Garrett County Lighthouse as the Residential Director. Previous titles while employed at Garrett County Lighthouse included, Behavioral Health Technician-Crisis and Support Counselor II. Upon graduating with her master’s and becoming licensed in June 2017, Christy became employed through the Garrett County Health Department as the therapist for the Maryland Community Criminal Justice Treatment Program and provided therapy to inmates at the Garrett County Detention Center as well as at the Garrett County Behavioral Health Clinic. Christy completed her advanced field placement at Mountain Laurel Medical Center from Summer 2016 to Spring 2017 and was excited to have the opportunity to return as an employee. Christy also currently works part time for PrimeCare Medical at the Garrett County Detention Center providing crisis counseling and group therapy to sentenced inmates. Christy is a native of Somerset County, Pa but relocated to Garrett County, MD in 2014. She enjoys all the outdoor activities the area offers that she can enjoy with her rescue German Shepherd Dog. Christy enjoys being able to provide therapy and support to those typically underserved in the community.
Rob Funk, LMSW
Rob Funk, LMSW joined Mountain Laurel in February 2018 as a Behavioral Health Provider. Rob is a licensed master level social worker in the state of Maryland. He completed his undergraduate work at West Virginia University in 2001 and received his Master of Social Work from Boston University in January 2019. While engaged in his graduate studies, Rob worked as a behavioral health technician-crisis at Garrett County Lighthouse. Rob developed his clinical skills while completing both his foundation and advance clinical placements at Mountain Laurel during the course of his graduate studies. Rob prefers to practice within a cognitive framework but employs techniques from other therapeutic perspectives when situationally appropriate. Robert is a native of Kingwood, WV and has spent the entirety of his human services career in the Garrett County area. Rob and his wife (who also works as a social worker) reside in Albright, WV with their three school age children. Rob enjoys being a member of the behavioral health care system in Garrett County and enjoys working with team members within the Mountain Laurel system and in the community to improve patient outcomes.
Kelly A. Bailey, PhD
Kelly started working at Mountain Laurel Medical Center as a Licensed Psychologist in January of 2019. She is enthusiastic and primed to be providing psychological services to patients of all ages in a rural, primary care setting. After growing up in NH, Kelly attended Ithaca College in upstate NY, and graduated with a degree in Psychology and a minor in Gerontology. An initial heartfelt experience on a college research team that investigated empathy and wisdom among older adults influenced her pursuit of more education and research experiences to inform her future clinical practice. She obtained her Master’s degree in Psychology from San Diego State University, where she taught Developmental Psychology, and worked in several research labs at SDSU and UCSD, on topics such as depression, anxiety, social anxiety, and mental health within pediatric primary care. She then moved to Morgantown, WV, where she pursued her PhD at West Virginia University. In the Appalachian area, she gained more college teaching experiences, and practice experiences within community settings (e.g., children in foster care placement) and three college counseling centers. As a positive psychologist, she initiated dissertation research on happiness interventions among smokers with depression and pursued training to become a Gallup-Certified Strengths Coach. She loves to integrate strengths-based approaches to help patients achieve psychological well-being and to provide them with lifelong mediators of change, for example, tools to increase psychological flexibility and commitment to values. Dr. Bailey also provides psychological testing evaluations for children, adolescents and adults in order to inform behavioral and psychiatric treatment. She loves spending time with her husband and their three children, especially in the outdoors. One of her proudest accomplishments is having thru-hiked the Appalachian Trail, which has influenced her worldview both personally and professionally ever since.
Mary Pat Trumbule, LMSW
Mary Pat Trumbule, LMSW joined the Mountain Laurel Medical Center family as a Behavioral Health Consultant and clinical therapist in June 2019. She earned both her Bachelor of Arts and Master of Social Work degrees from West Virginia University. In 2018, Mary Pat received specialized training in Trauma therapy. She has provided services to clients of all ages and stages in life. Her focus is providing therapeutic services to individuals often marginalized in society. Mary Pat is a native of Allegany County. She currently lives in Lavale, MD with her husband. They are the very proud parents of two adult children, their partners and several “grand puppies”.
